The Jewish Home provides careful attention not only to its residents, but also to their families.
Our social workers work with residents and their families to meet their psychosocial needs, and especially, to help residents accommodate to a new living environment. We hold regular Care Plan meetings where our families talk about their concerns and needs with our staff. In the same meetings, staff, which often includes a social worker, the resident’s charge nurse, department directors and the residents themselves discuss how the residents are managing in their various therapies and activities. Care Plan meetings are a superb forum in which our staff can assist families to deal with the aging and illness of their loved ones.
At the same time, our conversations with family members enhance our understanding about the worries and difficulties families experience when they place their loved ones in skilled care.
